Passport

  • Original passport issued within 10 years
  • Valid for 6 months beyond the last intended date of departure from USA
  • Should contain at least 2 blank visa pages
  • Should have no alteration or handwritten amendments in the data pages

Photograph

  • Three recent colored passport size photographs (35 mm wide by 45 mm high without border) and taken within the last 6 months
  • With 50% - 69% of the image's total height should comprises of the face
  • Taken against a plain white background

Invitation

  • In case applicant is being invited then the valid Passport/Green card copy of the invitee along with their financial documents & Invitation letter mentioning purpose of inviting

Proof of sufficient funds for undertaking travel

  • Applicant needs to provide last 6 months bank statement signed and attested by a bank official in case the applicant is employed else need to provide bank statement of the person sponsoring your trip
  • In case applicant is self employed then company's 6 month bank statement is also required
  • All bank statements must be dated within the last 1 week

ITR

  • Income tax return assessment for the last 3 years in case the applicant is employed else need to provide income tax return of the person sponsoring your trip
  • In case applicant is self employed than he also needs to provide company's last 3 years of ITR until it's a proprietorship firm

NOC

  • NOC from Employer (if employed) along with ID card copy / Business card
  • NOC from School / Institute along with Xerox of ID card (in case of student / minor)

GST Copy

  • If Self Employed then applicant need to provide a GST Copy

  • All Indian passport holders require a valid visa to enter USA.

Similarly, if you’d like to visit someone who is admitted to any hospital in the US, a combination visa is required. Please note, you will only be permitted to stay in the country for a maximum period of 6 months,
